Right, Kagome did confuse him I believe its because she was Kikyo's reincarnation , but when Kikyo was even a zombie, he loved her and he has good reasons to do so. InuYasha would have never chosen her if Kikyou were still alive: their love was deeper. Kagome, being the closest person resembling Kikyo and was alive , just poorly replaced her.

Inuyasha is and was always there to protect both of them to the best of his ability. He feels he didn't try hard enough to save Kikyo's life, but he gave it his best. When she come to Inuyasha's world, Kagome traveled a lots with Inuyasha before seeing "his attacking". They claim nothing about their positions to each others not the same as Kikyo's case.

And the result was Kikyo's death. So yes, he absolutely didn't want to make that mistake twice.
